Epic will provide you with the Xbox and PS4 versions of UE4 if you are a registered developer for either.

In almost all cases, you need to have a legal registered entity of some kind to get a license. If you can't do this, you need to work with someone who can. Sorry.

For Xbox One you can get started here: http://www.xbox.com/en-GB/developers/id

For PS4 you can get started here: http://www.playstation.com/en-us/develop/

To receive your console code access, you will need to contact the company you wish to develop for and register for access to their SDK. Then, you will need to email consoles@unrealengine.com and let them know that you have been approved. They will prompt you to send them some information so that they can verify you. This will not change the cost of your subscription. Please let me know if you have any questions.
