How to make a bullet HOLE in the wall

Hi, I would like to spawn a hole in an object in the world, so you can see through the object, if possible, remove the colliders from the hole, so, in case of a bigger hole, you can walk through.

If people need an answer, you can remodel your mesh at real time with vertices. I got the concept from the Siege destruction panel at GDC.
