Physics Asset Generating Colliders in Wrong Positions

I may not be rigging things properly, but I can’t seem to create a physics asset for my skeletal mesh that properly positions all the colliders based on my character’s bone locations. As you can see from the screenshot, most are positioned properly, but some are just way off.

Does anyone know what I might be doing wrong, or how I might be able to generate the physics asset more accurately?

I should also note that the character’s bones are not vert weighted, which may be causing this issue?
