Vulkan Pipeline / Rendering Problem

Hello.

I am having a issue with Vulkan SM4 on Windows when I travel to a map then leave that map and travel back to the main menu. Weird Rendering Glitches happen.

Vulkan SM4 Main Menu (After Corruption Happens when returning from a map)

Vulkan SM4 Char Select (After Corruption Happens when returning from a map)

Vulkan SM4 Main Menu (Another Corruption Glitch when returning from a map)

Vulkan SM4 Char Select (Another Stacked Corruption Glitch)

Vulkan SM4 Map (Last 2 glitches / blends have stacked across map travel)

Vulkan SM4 Main Menu ( A new corruption glitch )

Please Note: The character selection level uses a render target, this might effect results.
