Volumetric Lighting and Spotlight Help

The Short Story
I would like to create a volumetric spotlight with clipping control of the visible beam

The Long Story
I am able to create spotlights with volumetrics, however, lighting and optics in the real world don’t usually start at a single point in space (img 1). Most of the time light is manipulated with optics and comes out of a fixture at a large diameter (img 2). I haven’t found any way to do this in UE4. Am I approaching this all wrong? Should I be creating geometry and a material for the light beam? If I go down that path what will happen with volumetric shadows on geometry and if I ever use a gobo (img 3)?
